Venous Stasis Ulcer Biopsy, Bleeding Control

Bleeding control using a topical hemostat after biopsies on a venous stasis ulcer.

 

This is a 69 year old diabetic who already has an AKA on the right side. She also has chronic kidney disease, peripheral vascular disease and she is on dialysis.

She has these venous stasis ulcerations, lateral ankle, posterior ankle, and there’s another one on the medial side as well. These were biopsies that we had performed and you can see these were two millimeter punch biopsies that were taken. She is on Coumadin and aspirin therapy as well.

You can see the chronic stasis changes in hemosiderosis, lipodermatosclerosis of her legs. You can see after these biopsies it is bleeding quite well. I’m just showing you the stasis changes on the legs, indicative of her severe venous disease.

Then we apply the Omni-Stat over this area so that we can gain hemostatic control because she was very afraid of going home and bleeding through her dressings. She can only make it to see us once a week because she relies on public transportation. So we applied the Omni-Stat to gain hemostatic control and then afterwards we applied Santyl, Promogran™ and Drawtex® to the wound and then multi-layer compression bandaging because she has the severe deep venous reflux and lymphedema as well.
